Health and Insurance Benefits

We strive to create a culture of excellence in everything we do, and that includes protecting the health and well-being of our team.  Following are highlights of the various health and insurance coverages available to employees of Cardno ENTRIX. 

Medical

Cardno ENTRIX offers two medical plans through CIGNA Health Insurance Company.  Both plans are open access plans in which referrals are not needed to see specialists.  Both include prescription coverage, preventive care, chiropractic coverage, acupuncture, and have an annual maximum of $2,000,000 for each covered individual.  Enrolled employees have access to a 24-hour nurse line, the Healthy Mother and Baby program, Well Aware Disease Management programs, Lifestyle Management programs (i.e. smoking cessation, weight management), and Employee Assistance programs, which provide confidential counseling and referrals for work/life balance issues. 

Dental/Vision Discount

We offer dental coverage through MetLife.  Coverage includes full coverage for two routine examinations (at usual/customary charges) every 12 months with no deductible.  The plan also offers coverage for minor and major services at prorated rates.  Our dental plan also provides orthodontia support up to a lifetime benefit of $1,500 per child.  Employees enrolled in the dental plan are automatically enrolled in the vision discount program which provides discounts for lenses, frames, contacts, and corrective surgery.

Flexible Spending Accounts

Cardno ENTRIX offers three Flexible Spending Plans—one for Medical, a second for Dependent Care, and the third for Qualified Transportation Benefits.  Employees can set aside up to $3,500 annually in pre-tax dollars for medical, $5,000 annually for dependent care, and $230 a month for both public transportation and parking. 

Short and Long Term Disability

Cardno ENTRIX provides both Short- and Long-term Disability insurance at no extra cost to the employee.  Short-term disability provides benefits when an employee is unable to return to work after 7 consecutive days.  The duration of benefits is dependent upon an employee’s individual situation and can continue for up to 25 weeks.  Employees are eligible to receive 60% of their weekly salary (to a maximum of $2,500 per week).  After 180 days, depending upon an employee’s individual situation, long-term disability may begin.  Long-term disability provides loss of income protection when an employee becomes totally disabled and unable to work. An employee is eligible to receive 60% of his or her monthly base salary (to a maximum of $10,000 per month). Payments continue during the disability period until at least age 65.

Life Insurance and Accidental Death and Dismemberment Insurance

Cardno ENTRIX provides Term Life insurance and Accidental Death and Dismemberment (AD&D) through The Hartford Financial Services Group, at no cost to employees.  Coverage is one times base salary (up to $200,000 maximum).  Additionally, if our employees want additional coverage, they may purchase Optional Life and AD&D coverage up to five times their annual salary (not to exceed $500,000).  If purchasing extra individual coverage, an employee may also purchase additional coverage for a spouse, up to 50% of your election, not to exceed $250,000 and children either $5,000 or $10,000.

Aflac Voluntary Insurance Coverage

Employees also have the option to purchase Supplemental insurance through Aflac, and a variety of policies are available, such as Accident, Hospital, Cancer and Specified Health Events.  Unlike traditional medical insurance which pays for health-related expenses like doctors and hospital visits, prescription medicines or medical procedures—supplemental insurance pays cash that can be used however an employee chooses.  For example, supplemental benefits can be used to cover everyday, out-of-pocket expenses—like groceries, rent, general household bills, that can really add up should someone get sick or injured.
